If you’re on a fertility journey, you may have heard about the connection between thyroid health and pregnancy. It’s true: the thyroid gland plays a key role in reproductive health. But recent research has updated our understanding of how much thyroid issues actually impact fertility and early pregnancy.

Why Your Thyroid Matters in Fertility

  • The thyroid produces hormones that regulate metabolism, energy, and reproductive function.
  • During pregnancy, the body’s need for thyroid hormones increases, especially in early pregnancy when the baby relies entirely on the mother’s hormone supply.
  • When the thyroid is significantly underactive (overt hypothyroidism), the risk of miscarriage and pregnancy complications increases.

However, not all thyroid imbalances are harmful.

Many women with slightly raised TSH and normal thyroid hormone levels conceive naturally and have healthy pregnancies without medication.

What Recent Research Tells Us

Subtle Thyroid Changes Often Do Not Affect Fertility

Large cohort studies show that women with TSH levels on the higher end of the normal range (for example, 2.5 to 4.0 mIU/L) have similar chances of conceiving as those with lower TSH.

This means that a TSH slightly above 2.5 does not necessarily indicate a fertility problem.

Findings from UK fertility clinics show:

  • 4 to 5% of women have true thyroid dysfunction
  • 10 to 15% have thyroid antibodies, similar to the general population
  • TPO antibody-positive women are slightly more common in female-factor infertility, but most remain euthyroid and can still conceive

Treating Borderline Thyroid Issues Is Often Unnecessary

In the past, many women were prescribed thyroid medication for any TSH elevation. But new, high-quality evidence shows:

  • Treating mild subclinical hypothyroidism (TSH elevated but FT4 normal) does not improve fertility
  • It does not reduce miscarriage risk in the general population
  • Levothyroxine is not recommended for TPO-positive women if thyroid function is normal

What Do Guidelines Recommend?

  • Screen only women with clear risk factors
  • (e.g., known thyroid disease, autoimmune disease, recurrent miscarriage)
  • Treat only:
    • Overt hypothyroidism
    • Severe subclinical hypothyroidism (TSH >10 mIU/L)
  • Do not aim for TSH <2.5 unless the woman already has hypothyroidism or severe SCH
  • If trimester-specific ranges are not available, an upper TSH limit of around 4.0 mIU/L is acceptable

What This Means for You

1. Don’t Panic Over Slightly Abnormal Lab Results

If your FT4 is normal and you feel well, a TSH slightly above 2.5 is not usually concerning.

Current guidelines support using a higher upper limit (close to 4.0 mIU/L) rather than treating all cases above 2.5.

2. Screening and Treatment Should Be Personalised
